Community Information Exchange Summit: Building Better Connections for Better Care

Persistent health disparities and uncoordinated access to resources to support unmet basic needs have challenged the St. Louis region, manifesting in problems like high infant mortality, steadily increasing cases of type 2 diabetes or opioids addiction, high breast cancer death rates in minority women, shorter life expectancy for minority males and growing poverty rates amongst other social concerns. In the past year, area health care and social service agency experts have begun building a tool and a new trajectory for partnership designed to finally move the needle on such disparities and build stronger connections to move our neighbors (and ultimately our region) from crisis to stability.

On Dec. 13, hear how the St. Louis Community Information Exchange (CIE) will harness information to focus existing health and social outreach efforts, break down silos between agencies, and, most importantly, give neighbors, patients and clients access to the range of services and supports they need. Learn about work being done to adapt the information exchange model used successfully in other cities to the specific challenges of the St. Louis region. Get a demonstration of how the St. Louis CIE will be used to connect patients and clients with already available health care, community supports and social services in real time during a single encounter, and learn how it holds the key to solving problems many had considered intractable.

This event is brought to you by the CIE Visionary Leadership Team . The CIE is a network of partners focused on improving the health and wellbeing of people in the St. Louis region. Using a shared technology platform, partners will be able to share information and coordinate efforts to maximize resources in the delivery of holistic, person-centered care.
